Let the office know which service interests you so the team can help with the next step.
Visit Our Davenport Office
All appointments take place at 2211 E 52nd St, Suite D, Davenport, IA 52807, behind Syndicate Hub. Patients visit from Iowa and Illinois communities throughout the Quad Cities. See location and directions before your visit, especially if this is your first appointment.

- Monday: 8am-10am, 3pm-6pm
- Tuesday: 3pm-6pm
- Wednesday: 8am-10am, 3pm-6pm
- Thursday: 3pm-6pm
- Friday: 8am-10am
Call the office to arrange an appointment during these hours. An online request is not a confirmed appointment until the office contacts you to schedule.
Before Your First Visit
The New Patients page includes information about insurance, fees, and getting started. Adult and pediatric intake forms and the Functional Assessment Questionnaire are available on the paperwork page. If you are unsure which form applies to your visit, ask the office before completing it.
For insurance or payment questions, call with the details of your coverage. The office can discuss available payment options and the next steps for chiropractic or functional medicine appointments. Please do not include insurance identification numbers, detailed medical records, or other sensitive information in a general website message.
Messages and Urgent Concerns
Use the General Contact Form to provide your contact details for non-urgent follow-up. To send a question, comment, request for service information, or newsletter request, email the office with a brief description. For an existing appointment or a time-sensitive scheduling question, call the office directly rather than relying on an online request.
